The doctors and staff at the Hilltop Medical Practice in Harlesden are proud to offer the highest standard of patient-centred healthcare.  We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

Hilltop Medical Practice is planning to merge with The Stonebridge Practice. We are keen to hear your views and address any questions that you may have. Care Quality Commission

We had our second inspection on  23rd February 2016. Many thanks to our Patient Participation Group who attended to represent our patients and give their feedback. 

We are proud to present their findings which found us attaining a GOOD standard in ALL PARAMETERS.

How Can I Access Care ?

Online access

The Practice is pleased to announce the introduction of Patient Access. 

Patient Access includes a number of features, including:

  • Checking, booking and cancelling appointments.
  • Checking your medication and ordering repeat medication.
  • Updating your contact information, including mobile phone number and email address.
  • Accessing your Medical History, list of allergies, medications and investigations

 

All from your home computer, tablet or smartphone.

Need urgent but non-emergency help when the GP practice is closed?

If you have an urgent medical problem which cannot wait until the surgery re-opens please call the NHS 111 service by dialling 111.

NHS 111 is free to call from both landlines and mobiles.

When you call the NHS 111 Service you will be asked some questions about your symptoms so that you can be directed to the local service that can help you best.

If, for any reason, you are unable to access NHS 111 by dialling 111, please call 0203 402 1111 instead. Calls to this number are charged at your network's standard rates.

Face to Face Advice :

Hilltop Medical Practice works with local Practices to provide face to face care for all our patients 24 hours a day, EVERY DAY! You may not be able to see YOUR chosen doctor but you will not be sent away.

GP Access Hub

GP Access Hubs

GP Access Hubs have appointments everyday (8am to 8pm)and in the weekends (timings under review)  for those who can not come during the day due to other commitments. Appointments can be made by our receptionists at a time that suits you. These Hub appointments may be sited at Central Middlesex Hospital, Willesden Centre for Health and Care or Wembley Centre for Health and Care depending on the day of the Appointment. Our receptionist will ask for your consent for the Doctor at the Hub to see your medical records.

A&E services are changing at Central Middlesex Hospital and Hammersmith Hospital

On Wednesday 10 September 2014, the A&E departments at Central Middlesex Hospital and the Hammersmith Hospital will close.

Urgent Care Centres at both sites will continue to treat patients with minor illnesses and injuries that are urgent but not life-threatening 24 hours a day, seven days a week

Adults and children can use the Urgent Care Centre for:

  • Sprains and strains
  • Minor burns
  • Cuts
  • Minor infections
  • X-rays

 

Urgent Care Centres (UCCs) are run by experienced GPs and nurses with skills in urgent care.  UCCs are for patients who cannot wait for a GP appointment, but do not need the emergency treatment provided by an A&E.  You do not need an appointment to be seen at a UCC.

In an emergency, call 999.

Call NHS 111 if you need urgent medical help or advice, but it’s not a life-threatening situation.

Chlamydia Screening

We offer free chlamydia screening (testing) to all men and women aged between 16 - 24.

Most people don't know they have chlamydia and that it can cause pain and infertility if left untreated. 1 in 10 people have chlamydia - but not all have symptoms. Chlamydia testing does not require booked appointments, examinations or blood tests- just a simple urine test.  Please enquire at reception

Carers

DO YOU LOOK AFTER SOMEONE WHO IS ILL, FRAIL, DISABLED OR MENTALLY ILL?

Do you look after someone who is ill, frail, disabled or mentally ill? If so, you are a carer.  We are interested in identifying carers, especially those people who may be caring without help or support.  We know that carers are often “hidden” looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a carer.

We feel that caring for someone is an important and valuable role in the community, which is often a 24-hour job that can be very demanding and isolating for the carer.  We further believe carers should receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits and respite care and not least, a listening ear when things get too much.

Named GP for patients aged 75 and Over

From April 2014, all patients over 75 will have a named GP who will have overall responsibility for the care and support that our surgery provides to them.  They will also work with other relevant health and care professionals, who are involved in their care, to ensure that the care package meets their individual needs.

BEES Newsletter

Our Health Partners Forum takes place on Wednesday 31 January 2018

Come along to the Health Partners Forum on Wednesday 31 January where we will discuss Urgent & Emergency Care services in Brent and give an update on mental health services from the October meeting.  We will also talk about long term conditions (LTCs) such as respiratory conditions and diabetes, end of life care, older people, children & young people and discharge from acute services.

The Health Partners Forum is a public event which brings patients together with representatives from the NHS, the local authority and voluntary sector.

At the forum, patients are given a chance to hear about important health and social care issues happening in the borough. It also provides an opportunity for patients to share their thoughts and experiences on health and social care in Brent.
